Probably a corrupt world. In the past we have not attempted to fix MZX's treatment of corrupt files because the way its world/sav loader is designed, it doesn't check for any C stdio errors. Making this code do so involves rewriting it.
I'll leave this bug open anyway, in case somebody gets around to it. It's definitely a real issue.
