Well-well… Three guesses as to how I found this.

Issue: "mod fade" Works Only Once When Triggered By "justentered" Unless Preceded with a "volume" Command
Versions Affected: 2.02 onward
Introduced in Version: 2.02 (or whenever "mod fade" was introduced…)
Operating System: DOS (2.02+), Windows, presumably others

Further Details
I'm not sure if I'm explaining this correctly, but basically, if you have two boards which both have "mod fade" code that triggers via "justentered", they simply don't work as intended. Whatever board was entered last will retain its volume despite any "mod fade" code.

This seems like another one of those weird technical oversights that never got caught. If I had to guess, each board has its own "volume" counter, but said counter doesn't get updated when transitioning between boards. Because of this, any "mod fade" commands being utilized via "justentered" label can't do their job because the local "volume" counter hasn't changed one iota. Or… something.
